A: Publix Pharmacy Direct Care is an easy, convenient way for you to get your diabetes testing supplies delivered right to your home.

Q: Is there a charge for this service?
A: No. Membership is free and there is no charge for shipping.

Q: How do I enroll in Direct Care?
A: There are two easy ways to enroll:

1. Call us at 1-800-PUBLIX RX
2. Fill out the forms provided online at www.publix.com/rxdirect and either drop it in the mail or fax to us using the information provided.

Q: How does Direct Care work?
A: You will need to fill out some forms, including:

• Enrollment Form
• Acknowledgments and Authorizations for Assignment of Benefits and Use/Release of Medical Information (if eligible for Medicare)
• Credit Card Authorization Form (for payment of applicable co-payments, co-insurance, or deductibles)

Q: How long will it take to receive my order once I have enrolled?
A: It takes approximately two weeks for the first order to be processed and shipped. If you need supplies right away, it is best to take care of your order at your local Publix Pharmacy. You can then enroll for the future and our Direct Care Staff will take over from there.

Q: What if I have questions about the program, about when I will receive my supplies, or other general questions about diabetes?
A: You should call us at 1-800-PUBLIX RX. Our support staff will be on hand to answer questions during regular business hours. After hours, you can leave a message, and our support staff will respond the next business day.

Q: As a seasonal customer can Direct Care mail to me during the months that I am living outside Publix's market area?
A: Yes! You can continue to receive your testing supplies during those months. You simply need to alert us to the change in address by calling us at 1-800-PUBLIX RX.

Q: What if I want to continue to receive my diabetes testing supplies at the Publix Pharmacy?
A: Enrollment in Direct Care is optional. You can always continue to receive the same great service from our Publix Pharmacies if you choose.
